Ms. [redacted] was scheduled for a Nuclear Treadmill.  When the patient is scheduled for this test they are given an instruction sheet for the testing.  In this it states that there is a $250.00 fee for cancellations that are not made 24 hours prior to the test due to the fact that we cannot...

reuse the medication that is being ordered for the testing.  We did not receive a cancellation in that time frame so the patient was charged the $250.00.  I spoke with [redacted] at Kings Credit Bureau last week regarding this womens account and due to the fact that this is from 2012, we were not scanning these documents into the patient's file at that time and we were not keeping them for more than 3 years we told [redacted] to take it out of collections because we do not have the form with her signature that she initially signed. But again the patient is fully aware at the time the service is scheduled what needs to be done.  She says she let us know our front office says we were not notified in time.   Thank you,   [redacted]
