Ms*** is complainingbecause *** ***-Virginia refused to honor her contract with them. I am not currently a *** provider, howeverthe carrier has a contract with Ms***, and the fact that I am now “outof network” does not relieve *** of its obligation
to Ms***. Ms***’s assertionthat Aspen Valley Prosthodontic Dentistry, PLLC has an inactive Federal Tax ID #is simply false. At the time Ms***first came to the office, March 19, 2014, AVPD, PLLC was an in-network providerfor *** *** Colorado. Reviewinginsurance records, we found that the office manager at that time, ***, apparently never filed any insurance claim documents for Ms.***. This was discovered by *** in June Aspen Valley ProsthodonticDentistry, and specifically Mrs*** has been accused of incompetence. In fact, the former office manager, Ms.***, was certainly negligent in failing to file several claims with***. Until Ms*** left thepractice, we were unaware of her negligence. Mrs*** informed Ms***, by phone, that we were no longer “innetwork” with ***, but were “in network” when she initially came to theoffice. Had the claim for Ms***’s*** coverage been filed in a timely manner by the former office manager, thisunfortunate situation would not have occurred. Nevertheless, *** has a contract with Ms***, which itis failing to honor. At this time, ***’sbreach of contract with her would seem to be the entity upon which she shouldconcentrate her efforts. Ms***might want her employer to intervene with *** on her behalf. Aspen Valley ProsthodonticDentistry, PLLC provided dental services for Ms*** in good faith. It is Ms*** from whom we will continueto seek payment for services provided as our “contract for services” is withMs***, not with ***. We willaccept the amount that *** reimburses for Ms***, providing that ***issues the payment directly to Aspen Valley Prosthodontic Dentistry, PLLC., andwe will then write off any balance due. Finally, Ms*** seemsintent on vilifying Mrs***. Heraccusations thereto, I find inconsistent with Mrs*** performance as anemployee. I find it unacceptable andunfair that I am apparently restricted from identifying the complainant in myresponse, while her complaint clearly vilifies Mrs*** as well as mybusiness. I suggest you contact mepersonally prior to publishing the complaint and my response. *** ***, DDS

Revdex.com,I have returned [redacted]'s calls on three occasions, leaving messages each time. I have suggested that, since we cannot seem to connect, she can contact me through my personal email. My tax I d # has not changed and [redacted] Colorado has it on file as do claims submitted on Ms [redacted]'s behalf. [redacted] Virginia can issue a check to Ms [redacted]. I will accept the amount of payment by [redacted] to Ms [redacted] as long as proof thereof is documented as being the [redacted] payment in full. I repeat: I am not ,and have never been responsible for [redacted]'s contract and issues with Ms [redacted]. My contract for services as a private practitioner is/was with Ms [redacted]. I do not render services for or through [redacted]. This is my final response to this "complaint". Again, [redacted] has/had a contract with Ms [redacted] and she must seek satisfaction from [redacted] has the information it needs from me.[redacted], DDS

[redacted]’s Tax ID number has never been given to [redacted]
Dental of Virginia – they do not have one on file, so it needs to be updated
within their system.
[redacted] canceled their contract with [redacted] on
March 1, of 2014, although his office claimed to be in-network and allowed me
to come to his office as an in-network patient.
Mr. [redacted] seems to be unaware of this fact.
[redacted] of [redacted] has tried to speak to [redacted] and has left ten messages so far (documented within their system). Messages he is also unaware of.
[redacted] of [redacted] will be trying to contact [redacted]
again Friday 12/12/2014 morning at 9:30am and again on Monday 12/15/2014.

[redacted]’s contract with [redacted] (Aspen Valley Prosthodontic
Dentistry, PLLC) states that out of network payments must be sent directly to
the patient first.
If [redacted] can please speak with [redacted], update
his tax ID within their system, [redacted] will send me an out-of-network
check per his original contract, which I am happy to sign over to him. The difference of any balance due must be
written off - I will not pay any out-of-network charges. Than we can finally clear up this matter.
