Review: Good afternoon Revdex.com, I'm writing because the Associated Clinical Laboratories located on Liberty Street in Erie, PA lost my bloodwork on two different occasions this year. I had a bloodwork drawn in February and the lab lost it, so it wasn't processed. I filed a claim with UPMC Health Plan and was told that the supervisor, [redacted], apologized for the inconvenience. They told me to come back and they would draw the bloodwork again. I went back to the same lab in June (either 7th or 14) and when I didn't receive the results for this screening, I called [redacted] and again, she said they can't locate the bloodwork again. I filed a complaint with UPMC Healthplan for this event, too.Desired Settlement: I do not know what ACL can do for me, if they lost the bloodwork, but I would like a complaint on file. The lab should be able to account for the bloodwork that is drawn. Losing it once, is bad enough. Losing it twice is unacceptable.

This letter is in response to the complaint that the Revdex.com received
concerning the blood testing that was mishandled by Associated Clinical Laboratories.
Associated Clinical Laboratories (ACL) strives for excellence in all aspects of our
business. Feedback from our clients and patients is crucial in providing us with direction
for continuous improvement. Please note that it is the policy of Associated Clinical
Laboratories to investigate each complaint thoroughly.
An investigation was conducted regarding this incident. The individual had their blood
collected in February and again in June at our ACL Patient Service Center.
The specimens were misdirected on both occasions~ The investigation revealed that the
phiebotomist failed to recognize that the patient was part of the UPMC Healthplan
Wellness Screening program. Our internal process resulted in the patient’s specimens
being misrouted to an alternate laboratory for welliness testing. Subsequently the patient
testing could not be located. To prevent this from happening again the handling of
wellness screenings will be reviewed with the phlebotomists.
Associated Clinical Laboratories makes every effort to ensure that we provide our
patients with high quality service. Again, I apologize for any inconvenience this has
caused you.
