Dear ***, It was a pleasure speaking with you yesterday in regards to resolving the billing issue with *** *** As we discussed, I left multiple messages for *** ***, but we have not received a return call After investigating the matter, it appears *** *** received an
Explanation of Benefits (EOB) from his insurance company and may have misinterpreted this as a final statement I have attached the billing guide that would have been provided in the patient collection package at the time of the testing In addition, we do ask our health care providers to explain our billing process or have the patient call us directly. Assurex does bill the insurance company for the full cost of the test, but we only hold our patients responsible for a typical co-insurance amount of $or less depending on their income level Assurex also offers our Financial Assistance Program to our patients *** *** will receive a statement once we have exhausted our efforts with his insurance company The statement will be adjusted to $regardless if the insurance company pays or denies the claim Assurex also provides the patient an application for our Financial Assistance Program with that statement If *** *** has a household income of under $75,000, then we would be able to adjust that balance depending on his income level *** *** will need to complete the Financial Assistance Program Application when he receives it, and return it along with a copy of his most recent tax return page showing his household Adjusted Gross Income (A.G.I.)In addition to our Financial Assistance Program we would also be able to offer *** *** a twelve (12) month interest free payment plan if the balance exceeds $150. Hi [redacted], Thank you for contacting us about the attached Customer Concern.  We regret that we have received such a complaint and that [redacted] had an unpleasant experience with our company. Our records indicate that this patient has Medicare insurance.  After speaking with [redacted]...

[redacted], I found that she received an explanation of benefits (EOB) from Medicare.  I explained to Ms. Khabgab that our records indicate that she currently has a zero balance.  We have a contract with Medicare and we do not bill the patient for any patient out of pocket. We are very sorry this patient had a reaction to her medication.  The Genesight test analyzes a patient’s DNA and helps their doctor get a better understanding of what medications will work best on the patient’s genetic makeup.  The GeneSight report is one piece of information that can be used to create an individualized treatment plan for patients.  Patients still must work with their health care provider to specific medication selections. We apologize for any inconvenience this has caused [redacted] and we appreciate having the matter brought to our attention.  Please do not hesitate to contact me if there are any questions or further concerns in regard to this matter. Thanks,  JANET H[redacted]GeneSightCustomer Service Solution Specialist
