documentation would be required in order to process claims.  Without specific information on the claims submitted, it is difficult to respond to this inquiry.  However, in general, common explanations for claims being denied or disallowed include the following:  the claim was not submitted with all necessary supporting information (missing radiographs, missing treatment records, etc.), the claim contains incorrect information, the services performed do not meet Delta Dental’s contractual guidelines, or the treatment is not a covered benefit the plan.  We suggest that the complainant refer to the reference sheet attached to the May 2017 letter when submitting claims to ensure that the necessary documentation is submitted with the claim.

Please type details here:]I have called Delta repeatedly and let them know that I am 100% aware of the required information they have requested to process claims and I am sending 100% of what they are requiring with every claim. They KNOW THEY ARE LYING when they say that the required information is not being sent. I have been in business for 38 years and I have never had this problem of insurance companies outright LYING to both me and my patients!They know the truth, and are outright LYING! I have had patients send the requested information themselves so that they can see that Delta is LYING! I really didn't expect this problem to be resolved by the company properly handling claims in a professional manner and I am totally disgusted with their response. This is a common problem of denials so they can save the company money and they know it.Thank you for your time.
