Delta Dental Plan of Michigan, Inc(NAIC #***) ("DDMI") is in receipt of the above complaint Upon receipt, our Customer Service department promptly researched this inquiry The complainant received a letter from DDMI in May providing procedure codes for which additional documentation would be required in order to process claims Without specific information on the claims submitted, it is difficult to respond to this inquiry However, in general, common explanations for claims being denied or disallowed include the following: the claim was not submitted with all necessary supporting information (missing radiographs, missing treatment records, etc.), the claim contains incorrect information, the services performed do not meet Delta Dental’s contractual guidelines, or the treatment is not a covered benefit the plan We suggest that the complainant refer to the reference sheet attached to the May letter when submitting claims to ensure that the necessary documentation is submitted with the claim

I have had patients send the requested information themselves so that they can see that Delta is LYING! I really didn't expect this problem to be resolved by the company properly handling claims in a professional manner and I am totally disgusted with their responseThis is a common problem of denials so they can save the company money and they know it.Thank you for your time Regards, [redacted]
