January 8, [redacted] was seen as a patient at Atlas Physical Therapy, PC from 1/31/to 6/4/14.His last three dates of service, 5/19/14, 5/29/and 6/4/14, were denied by ***, his insurance companyOur biller, [redacted] , filed an appeal of this denial with *** This appeal was subsequently deniedAn employee of [redacted] contacted and spoke with [redacted] on July 30, to notify him that our appeal was denied, The representative told [redacted] that she would contact [redacted] to see if there was anything else we could do to help him with his appeal [redacted] received a letter from ***, which is attached, stating that our appeal was denied and that the patient may file a personal appeal with ***A copy of this letter was sent to [redacted] by ***, as referenced at the bottom of our copy.On July 30, the representative from [redacted] sent a request to [redacted] via [redacted] to advise as to why the claim was denied.On September 26, [redacted] contacted Atlas Physical Therapy, PC and stated that [redacted] did not like something about his prescription from his doctor for therapy, so he asked his doctor to write a letter to *** [redacted] said he felt this should get the claims approved for payment.On November 14, [redacted] contacted [redacted] after receiving another bill he reminded [redacted] his appeal was ongoing at that time [redacted] called [redacted] back on November 14, and left a message on his answering machine saying that he still had an open balance, [redacted] , as a representative of Atlas Physical Therapy, PC exhausted ail available avenues to receive payment from [redacted] for [redacted] Our appeal was denied in July and it was [redacted] ’s responsibility to pursue a personal appeal after that date as referenced in the letter sent to both [redacted] and [redacted] from [redacted] Atlas Physical Therapy, PC could have demanded payment at that time and have [redacted] pursue reimbursement from ***, but instead allowed [redacted] more than a calendar year to pursue payment from ***When Atlas tried to contact [redacted] in September of to inquire about the status of his appeal both his cellphone number and home phone number in our records had been disconnectedAt this time Atlas Physical Therapy, PC sent [redacted] 's account to collections
