Axiom of Planters Walk

Axiom dentistry has failed to provide insurance company with the necessary information for the insurance to pay the bill and now they are billing meOn 3/19/14 my husband ([redacted]) had a build up done at the dentist. The dentist failed to send a detailed narrative to the insurance company. Claim was denied due to no narrative. I spoke with the billing person at the dentist ([redacted]) several times and was unable to rectify this. Several months ago, I spoke with [redacted] again. She told me they (axiom dentist) had filed the claim incorrectly and she resubmitted the claim as a "filling". By the time she had done this, it was past the 184 days allowed in which to submit a claim. Claim was again denied for "timely filing". The insurance company advised me to have the dentist submit an appeal to their quality control department with a detailed narrative of the build up and supporting X-rays. I have called the dentists office multiple times to give this information to [redacted] and I'm always told that she is out and even through I leave my phone number and information, I have never received a call back and I continue to get a bill from the dentist for the cost of the build -up on my husband's tooth. Desired SettlementI am not responsible for this bill of $257.40. If the billing department at Axiom dentist had done their job correctly, they would have been paid by the insurance company. The settlement I am seeking is for the dentist to appeal the bill, and if the claim is denied, I would like the balance written off since this balance is a result of the dentist failing to bill correctly (build-up vs filling) and they also failed to provide the insurance company with the detailed narrative that the insurance company requested in order to pay this bill.
