This place is by far the worst place ever! Came in to get Botox done, one employee told me the nurse was was at the office on Saturdays and Wednesday'sI was schedule for an appointment on Saturday, I come and I magically dissapiered from the scheduleThey told me they couldn't do anything about it because the nurse who does Botox isn't in office on SaturdaysSo I requested my money back they refused stating they don't give money backI tried to speak to a manager they refuses stating she was assisting a patientI called later that day, requested for a manager to call me back and never called me BackSo I had no choice but to take off work and scheduled my appointment two weeks outA week before my appointment they called me and told me the nurse is no longer here on Wednesdays and now is Tuesday's and Thursday'sThey have no type of customer service what so ever, the girl who answered the phone, is rude obnoxious and thy don't know what they are doing! do NOT waste your money on this place! I rather spend more money somewhere else, were I know I will receive excellent customer service!

Although I appreciate the willingness of the business to have me used the money I spent, I cannot accept this responseI attempted several calls to this business regarding my experience prior to contacting Revdex.com and my credit card company to try to clarify my experience and have a resolutionMy calls were not followed up properly, but now that a complaint is in place I received a call back from the business trying to have me come in for treatment saying that indeed there was a mistakeThat's the resolution I attempted to get at with all my calls initially to them and after not receiving an appropriate response I was forced to contact my credit card company, the Revdex.com and AZ attorney's General officeMoreover, I do not feel comfortable receiving treatment of a medical nature at a place where my medical chart appears to have been noted incorrectly and no effort was done to follow up with me regarding this or clarify what exactly happenedI do not see a difference in the amount, I would like $refundedI had paid for $for a syringe of Restylane that was not used and I paid an additional $to have this credit transferred to be able to purchase a Voluma which I never used either

I do not see a difference in the amount, I would like $600 refunded. I had paid for $200 for a syringe of Restylane that was not used and I paid an additional $400 to have this credit transferred to be able to purchase a Voluma which I never used either.

Attached are the acknowledgement, consent and invoice Mrs. L[redacted] signed prior to receiving her treatments. In it she acknowledges that we cannot give refunds, that additional treatments may be necessary, there is no guarantee of any treatment and results may vary.  Please review.

Mrs. L[redacted] purchased a package which included a V-Lift, Secret RF Treatment and 38 units of Botox.  During her 1st appointment with us she was administered 38 units of Botox.  20 Units where to be complimentary and 18 units was discounted as part of her package price.  At this point...

she has opted not to continue with treatments and stay with the 38 units of Botox already administered. As for the effectiveness of the treatment, all Botox in our office is administered according to manufacture standards.  Mrs. L[redacted] acknowledged in writing on a consent form that effectiveness of Botox varies from person to person and additional injections may be needed to achieve a satisfactory result. Our retail price per unit of Botox is $12/unit which totals $456.00.  Although we have a no refund policy, which Mrs. L[redacted] acknowledged in writing, we can refund the difference of the unused portion treatments which comes out to $888.

Regarding the complaint ID#: [redacted], we have issued a full refund on 4/14/2014....

I have had Botox treatments 15 times in the past 8 years. It has always worked to paralyze the muscles fully and has lasted three to four months. I have gotten a  wrinkle free forehead every single time, lasting 3 to 4 months, with only 24 units of Botox. I have two close friends who get Botox treatments due to frequent migraines that have not responded to any other medication. They get full paralysis of all muscles treated and their migraines fully abate. I have a co-worker who has benign tremor that effects her entire body. When she comes to work after treatment she has absolutely no tremor. 3 and 1/2 months later tremors begin to re-emerge and she then goes for another Botox treatment once again she is tremor free. Botox works.I am certain that what I was treated with at AZ Laser was primarily water because I look as if I have had no treatment. I have read many  negative reviews on [redacted] about  AZ Laser on [redacted]. In one review the reviewer states that an employee of AZ Laser went to another room to fill syringes for her treatments, the reviewer/customer went to look for the employee and when she opened the door she saw the employee at a dirty, rusty, sink filling the syringes with tap water.  I am demanding a full refund of my $1344.00 dollars paid on 04/08/2016.  This company is willfully fraudulent.
