Mrs*** phoned this office and requested her
husband’s social security number to get his insurance informationShe wanted
this office to fax her records to her, personally. After reviewing her dental history, I told
her she did not have any current x-rays (over months old)I also
told her
that we would be happy to send any dental information to her new dentist or they
could phone us with any questions.Azle Dental Care phoned and the only information given
was an insurance ID number, to verify Mrs*** insurance. She insisted on copies of her records and I told her I
would need to review them and that there could be a fee to duplicate her
records and x-rays (we do not email because the x-rays are not digital)I phoned her back and told her the only fee would be
for duplication of the full mouth x-ray taken July, She agreedShe
wanted this done by Monday (12-7-2015) and I explained that we would need time
to duplicateI phoned her on 12/04/and told her the copies would be ready
to pick up on MondayShe agreed to pick them upI phoned again on Monday
12/07/to let her know the duplicates and copies were readyShe said she
would pick them today (12/07/2015)I will mail copies and duplicates to Azle Dental Care
per Mrs*** requestI apologize for
