Hi, I'd like to resolve any ongoing concerns. We have not sent you a bill. We will not bill you for any non-covered services. We only call prior to testing if a patient will be billed for non-covered services. The consent form we received from you and your physician outlines this policy. If there was any miscommunication received in your doctor's office, we apologize and we would like the opportunity to remedy it for you. We've agreed not to bill you anything unless you receive a check from your insurance company intended for our services. [redacted], I’d be glad to help resolve your billing complaint filed on the Revdex.com website.    Please note that we have not billed you anything.  What you are seeing is the claim information on your Explanation of Benefits (EOB) that shows 8 procedure codes in your testing panel. ...

These totals are simply the list charges for the testing.  We accept the allowed rates that your insurance will assign to each procedure code.  You are not billed the difference between list charges and allowed rates.    If the EOB from your health plan shows that the testing has denied or that you will owe a large amount of money, please do not be concerned.  We verified benefits prior to testing, and several of the procedure codes are expected to be covered by your insurance.  If any of the procedure codes result in a denial, we will file an appeal and will need some time to finalize the insurance billing.  We do not bill patients for any non-covered (denied) services.    If the test is partially covered by your insurance company, we will bill you only up to the amount you were expecting.  If more clarifications is needed, please let us know.   Sincerely,Myriad Billing

the patient.  Your testing was covered by your insurance, but $375 was applied towards your cost-sharing amount (co-insurance and/or deductible).  Myriad is obligated to bill patients for cost-share (co-insurance and deductible) as outlined by your insurance policy.Your doctor's verbal quote of 100% coverage for our laboratory service was not accurate, and was not made known to us. However Myriad is committed to help any of our patients that receive an unexpected bill.  Your account balance has been reduced from $375 to $0 in order to honor the quote from your doctor.  I mailed a new statement reflecting this adjustment to you today.If anything more is needed, please reply or give our billing office a call at 1-844-MYRIAD9 (1-844-697-4239). Hi [redacted], I'll be glad to help resolve the concerns with your Explanation of Benefits (EOB).  We know that the EOB can cause concerns, especially when they show comprehensive genetic testing panels with several procedure codes being charged to your health plan.  These...

procedure codes represent important cancer gene testing that your doctor has requested.  Please know that Myriad is committed to securing the best possible coverage for your testing, and any denied charges are never billed to the patient without their specific consent. Some of the procedure codes on your EOB have a pre-authorization associated with them, and are preventative services.  This means that your health plan is expected to cover several of these procedure codes at 100% of the contracted rate.  You are not billed the difference between our list charges and the allowed, or contracted rates.  If any of the procedure codes deny as non-authorized or non-approved, etc. they will not result in a bill to you. If any of the procedure codes result in partial coverage with co-insurance and/or deductible showing on your EOB, this amount will be billed to you.  If this occurs, generous financial assistance options are available.  Myriad also has a program designed to reduce the out-of pocket costs for out of network cost share amounts, if this applies to your situation.  Our Myriad Promise program has a solution for nearly every billing outcome, and if you are billed we will work with you until you are 100% satisfied.  Please see more on this at www.myriadpromise.com.I am also happy to personally speak to you about any concerns you may have.  Please call me at 844-697-4239.
