encouraged to contact their insurance companies or EAP providers to discuss their coverage Although, we make every attempt to help patients navigate the complex insurance system, it is ultimately their responsibility to pay any unpaid balance due after insurance and EAP has been contacted. I am enclosing a copy of our Consent to Treatment, which every patient of Balanced, LLC is legally required to sign, that shows in writing where the patient is responsible for any fees not covered by insurance or EAP Patients are then asked if they understand the release before services are provided.
