5/26/2015 *** *** was seen by several doctors in my office between Novemberand June We have not seen him in three years We reviewed ***’s prior dentist notes and found that he was treated for a lower partial and upper denture between and 2006, he was very unhappy with both prosthesis’s and had multiple adjustments *** felt the appliances at Winston Salem Dental Care were not properly made The same complaint he made three years ago regarding this matter, and again today His medical doctor may have been treating him with multiple medications that can shed light some of his behavioral and physical issues We request he sign a release if he would like us to discuss his past and current medications and conditions with you and his medical doctors We absolutely did not recommend new appliances in Furthermore, we discussed the serious psychological and physical challenges created by medicine induced dry mouth, and the affect of his treatment for other possible nonphysical issues He failed to be forthcoming on his initial medical history form Furthermore, *** delayed treatment and refused the recommended reline of his current prosthesis He was informed that a new appliance would be difficult for him to tolerate and he would have to commit to wearing it and the typical sore spots and involvement of his medical doctor to manage his dry mouth. We discussed his psychological problems and concerns we had at length prior to initiating treatment Despite our warnings, Mr*** demanded new appliances agreed to our terms of giving up his old ill-fitting appliance, as it would prevent acceptance/ tolerance of a new appliance Furthermore, we discussed his medical condition and dry mouth and the involvement of his medical doctor and the potential changes to his medications that could be necessaryWe cautioned him that he might not be mentally ready to part with his ill-fitting appliance *** was given multiple try in appointments and final approval for the appliance He signed the record that he was happy with the shape size and shade of the teeth He was informed that by signing approval, the appliance could not be changed and we were unable to change the teeth or arrangement *** refused to wear the appliance and demanded his original back in just a few hoursHe stated that he changed his mind and was psychologically unable to go without his old denture and would simply not wear the new appliance for even one day The success of the new appliance is entirely based upon the patient’s willingness to give up the damaging old appliance, and wear the new one Simply, wearing the old appliance is a crutch and harms the gum tissue It is for a new denture to have sore spots and he was aware of this completely We answered all of his concerns*** then filed a complaint with Aetna three years ago Aetna was satisfied with our response, and the appliance was properly made and within the standard of care The matter has been closed for approximately years *** had the option for, and actually returned the denture in June However, he changed his mind quickly, and took the appliance back He has been happy with the lower partial denture that we recommended and we had made in April He has had the our upper denture for three years and there is no return We cannot accommodate his demands and particularly after three yearsI understand that he is frustrated, but there is nothing we can do *** has been to yet another dentist as his anger management issues and hostility prevented further treatment at our office Our office has a zero tolerance for violence in the workplace I have provided him with his records at no charge in After three years I cannot imagine *** making further complaint We have thoroughly addressed this with *** and his insurance company three years ago There is nothing further that I am able to add We are happy that he has found a new dentist, and wish him well However, we provided excellent appliances three years ago, and do not feel that his claims are rational at this extremely late date If you wish to discuss this further, please forward a complete release of information statement signed by *** *** in order to discuss his complete medical, medication and dental history in more detail We will need to include his doctors to discuss diagnosis and his treatment for each medication he takes as it most certainly has a bearing on this complaint. Review: I went to Dr Ballinger sometime ago He was my dentist. He had me a upper denture made. The denture didn't fit right in my mouth. I couldn't eat right with it in I couldn't even talk right with it in my mouth. He insisted they were ok and he wouldn't even work to try and get them right. I'm out $426.00 and I have and upper denture can't use. To add insult to injury he has kicked me out of his practice , which he done me a big favor I just think that he should be reported . He was very rude I have had to get another dentist which made me a perfect fitting denture. I think Ballinger should pay me my money back he can have the darn denture backDesired Settlement: I'm out 0f pocket a little over $426.00 dollars, Plus because he was so rude to me and I had to go to another dentist and have another denture made I feel he should pay for that expense too of $574.00
