with $760.30 being charged to the account ($760.30/4 months = $190.09 due for February & $190.07 due for March through May). This policy was issued on billing account [redacted].A downpayment of $168.93 was received on 02/13/2015, for the 2012 [redacted] policy. This payment was applied to the February amount due of $190.09, leaving $21.16 still due. An AFT Billing Statement was sent on 03/04/2015, advising a minimum payment of $612.26 would be withdrawn on 03/28/2015. (This statement was billing $21.16 which was still due for February on the 2012 [redacted] policy & the March amount due of $190.07 for a total of $211.23). Then on 03/27/2015, the 2012 [redacted] policy was transferred to its own billing account [redacted], along with the policy balance of $591.37. When a payment had not been received by 04/01/2015, a Past Due Billing Statement was sent asking for a minimum payment of $403.80 by 04/08/2015. This statement advised that if the $211.23 (past due amount for March) was paid by 04/08/2015, then the $192.57 (the minimum amount due for April) would be due by 04/28/2015. A payment of $42.30 was received on 04/02/2015, this payment was applied to the $211.23 still due for March, leaving $168.93 still due. Then a payment of $168.93 was received on the old account [redacted] on 03/26/2015. This payment was transferred to the new account [redacted] on 04/03/2015. This payment paid the 2012 [redacted] policy to 04/21/2015.On 04/27/2015, the 2012 [redacted] policy was cancelled per the insured's request "Other Insurance" effective 04/28/2015. A balance of $42.74 remains due to pay the policy to the cancellation date of 04/28/2015. Our records do not indicate any type of "change" was made to the 2012 [redacted] policy.
