Dear Ms *** ***,I am responding to complaint # *** .Patient Mr *** *** was seen in our office on July ,and had dental treatment for crown and a fillingFees were due the day of treatment but patient mentioned he wants to get reimbursed from his health savings account first
and will pay us later.Patient came back on August 09, and had crown seated and paid us with his health savings debit card for filling and crown.On Sept 13,we saw patient and adjusted the bite on the crown.Patient was not charged any crown adjustment fees.On Sept ,patient came in for his periodic exam and cleaning and said his crown is doing fine and there is no problem.Patient was mailed statement on Nov 08,2017 for the patient portion that insurance didnot pay for his cleaning visit on Sept 27,.Patient didnot Mail payment back from the statement.Patient came in on Nov 22, to start dental treatment ( crown) on another tooth.Fees were due the same day , but at time of checkout after his dental treatment was completed, patient said he wants to wait on payment till he gets reimbursement from his health savings cardPatient didnot mention any grievance with his charges at all .Patient came back on Dec 08, and made payment on his previous balance with health savings debit card and didnot mention any grievance.Since the complaint was received by us on Jan 03, ,we have attempted to call patient and have left messages for him to call us back so that we can give him a credit for dollars on his health savings debit card that we have written off for him but have not heard anything back from patient yet.Please let me know if you have any further questions in this matter.Thanking you,Sincerely,*** *** DDSBasswood Family DentalBasswood BlvdFort Worth,Tx 76137Ph
