I was not able to locate a record for a [redacted] in our patient accounts system or in the collection agency system I then ran a query from SQL for all records with the zip code (since she claims we have her “correct mailing address”) and out of the records returned, none of them had the name [redacted] or the PO [redacted] *** I also ran a query by her phone number – no resultsI checked with SCOA - since our anesthesiologists/CRNAs do services there - they do not have a patient by this name either.I would need a DOB and/or SS# to refine my search – or – the exact date of service as she only states “9/2016” as the “Hospital stay.”
