Thank you for contacting our office regarding complaint number [redacted] Our corporate office, US Imaging, Inc, and myself have thoroughly looked through this patient’s account and have taken this matter very seriously.Upon review of this account, patient was seen in our office for services and we were quoted from his insurance company an allowable price of $for that date of service As a convenience to our patients, we allow an interest free payment plan for them through a checking account or credit card We typically require at least 25% down, but to help this patient, we didn’t collect but $down at the time of service Patient signed a payment arrangement for $per month to be withdrawn on his credit/debit card.On 6/12, patient contacted our corporate office stating that on his explanation of benefits, his insurance allowed less than the quoted rate On 6/13, our corporate office refunded the $to his credit card However, due to the payment plans and primary account information being in two different programs, they did not notice that he still had a payment plan set up for one more payment left on his balance to come out on 6/20/ That payment did come out by mistake Once the error was realized, that money was refunded to the patient as well.We strive to help every patient and make the exam affordable and on a payment plan allowing the patient to be able to get their medical care they may otherwise not be able to afford Unfortunately this rare error occurred on Mr [redacted] ’s account We had resolved the issue with him and given him the full refund that was due to him prior to us receiving the complaint from Revdex.com.Thank you for bringing this concern to our attention If I can provide any more details or information, please do not hesitate to contact me.Melanie [redacted] Center Administrator/MarketerBay City Imaging / U.SImaging, Inc.(979)245-6660(979)245-(fax) [redacted] ________________________________________Attention: This e-mail is privileged and confidentialIf you are not the intended recipient please delete the message and notify the senderAny views or opinions presented are solely those of the author.________________________________________

and we were quoted from his insurance company an allowable price of $551.48 for that date of service.  As a convenience to our patients, we allow an interest free payment plan for them through a checking account or credit card.  We typically require at least 25% down, but to help this patient, we didn’t collect but $50 down at the time of service.  Patient signed a payment arrangement for $100 per month to be withdrawn on his credit/debit card.On 6/12, patient contacted our corporate office stating that on his explanation of benefits, his insurance allowed less than the quoted rate.  On 6/13, our corporate office refunded the $139.41 to his credit card.   However, due to the payment plans and primary account information being in two different programs, they did not notice that he still had a payment plan set up for one more payment left on his balance to come out on 6/20/17.  That payment did come out by mistake.  Once the error was realized, that money was refunded to the patient as well.We strive to help every patient and make the exam affordable and on a payment plan allowing the patient to be able to get their medical care they may otherwise not be able to afford.  Unfortunately this rare error occurred on Mr. [redacted]’s account.  We had resolved the issue with him and given him the full refund that was due to him prior to us receiving the complaint from Revdex.com.Thank you for bringing this concern to our attention.  If I can provide any more details or information, please do not hesitate to contact me.Melanie [redacted]Center Administrator/MarketerBay City Imaging / U.S. Imaging, Inc.(979)245-6660(979)245-6669 (fax)[redacted]________________________________________Attention: This e-mail is privileged and confidential.
