In review of Mr***'s account, he received services on 02/05/and we billed his insurance company (Blue Shield) in the amount of
$On 02/18/15, the claim was processed by Blue Shield and $was applied to his deductibleSubsequently, we billed him for the balanceIn fact, statements were generated for him on 2/27/15, 3/30/and 4/29/
Notes from our vendor state Mr*** would not issue a payment unless "deductible" was clearly marked on the statements
It is Mr***'s responsibility to understand his insurance coverage and limitations, including his annual deductibleAny misunderstandings of his coverage can easily be resolved by him contacting Blue Shield
We performed the service to him in good faithHe was not overcharged and his copay, if any, would have been collected at the facility (another company entirely)The debt is valid and is currently assigned to Creditor's Bureau USA

The previous response standsI am certain Blue Shield sent the patient an Explanation of Benefit EOB)statement, which is a clear recap of all charges and payments from the insurance companyAny part of the charges not paid would be indicated on the statementOur system does not put the non-payment reason on the statements which we conveyed to the patientI strongly recommend the patient contact Blue ShieldWe did not overcharge him and we sent him monthly statements before turning his account over to collections
