I visited this office on 04/and was seen by Brooke M***, she is the Psychiatry SupervisorI was there due to a severe stress, anxiety, I explained I needed to file for a LOA short term disability, she advised she would be filling out the paper workMy insurance declined my initial claim because Brooke never sent ANY documents after a month and a halfI filed an appeals which was declined AGAIN for lack of documents sent from BrookeSince my initial appointment she has NOT ONCE returned my callsI left her manager a VM as well and they have not returmed any of my calls
