I am very sorry this has happened to *** *** and Mrs*** ***. Errors were made the moment we received Mr*** ***’s death call. Information sheet was started and appointment was set for the family to come in and meet with the Funeral Director. We do
not want to make a mistake on the loved ones information. Our goal is to service familiesAfter meeting with the family paperwork is handed over to me, I certainly do not intend to make any mistakes. I fill out the death certificate online at Kentucky Vital Statics. Next, checked by director and forwarded to the doctor, which this was a deputy coroner, Rita Taylor.
As far as time line I have attached a print out from the website January 6, started the case, opened by director on January 13, 2015, (which the directors have rotation days off). Director forwarded to Deputy Coroner January 13, February 10, opened by coroner and rejected due to date of death was incorrect. February 10, corrected date and resubmitted to coroner. We missed a step and had to have Vital office reset on February 12, 2015. Submitted to coroner again February 16, 2015. Deputy Coroner accepted on February 18, 2015. Deputy Coroner forwarded to Vitals. February 18, death certificate was certifiedI cannot explain why it took the deputy coroner days to open and reject death certificateFamily came in and there were more errors. Spouse’s name was incorrect and his father’s initial incorrectAttached is the form, Kentucky Supplemental Report of Death that we had Mrs*** sign. This was faxed and we asked for more death certificates, no charge to family. Vitals office did not correct as written, only father’s initial was changed.
We are now waiting for the office of vital statics to call with what we need to do next. As Mr*** stated, this is unacceptable, I agree. We are doing everything to make this problem right.%

He wants his liner repaired, and I agree that it should be repaired, no argument.

The corrective action will be taken before the end of August, hopefully very soon, they have waited long enough.Tom C[redacted]
