We have tried to care for this patient many timesWe would love to take care of all the nursing and therapy needs that this client needsUnfortunately they refuse our nursing services for anything other than the IVWe do offer IV therapy for intermittent services, however the care this client
needs is for over 6-hours, which we then do not offer, and this is why he is using Brightstar services for hourly nursing careWe will be happy to care for this client and all his nursing and therapy needs, however other than the IV therapy, the client is not allowing us to do thisTherefore, after spending time explaining this to the client and family, we have to discharge since we are not able to treat other issues on this clientWe cannot ONLY do therapy, we are responsible for the each client as a whole Additionally, this client has not been discharged from our services as we were trying to communicate with his insurance company to see if we could come to some resolution for himThis was just discussed as our only option at this point until we can talk with the doctor and insurance company about the other health issues going onSkilled Home Health Services, as what our license entails, is temporary skilled care under the care of the physician with attainable goals in the near futureAccording to Medicare standards, a client will need to show an increase in ability and have shown some improvement to continue care with home healthOutpatient therapy is more appropriate in this case, as they focus on a higher level of attainable goals.
