To Whom it May Concern: Patient was seen in September and we billed their insurance provider that same month We were still waiting for payment in March and the patient was contacted for payment and subsequently paid the bill in full on March 17, Prior to contacting the patient for payment, we contacted their insurance company ( [redacted] ) whom informed our billing department that they were in need of additional information from the patient's primary care physician before they would process our dermatology claim from September A few days after the patient made their payment to us, they received an Explanation of Benefits from [redacted] dated March 19, that indicated they ( [redacted] ) needed additional information and also indicated the discount that would be applied to our dermatology claim The discount was shown to be $and we refunded that exact amount on check number # [redacted] dated 04/21/and mailed it to the patient's home address Since we have yet to receive any correspondence directly from [redacted] we were not aware of the discount applied to the claim (by [redacted] ) until the patient provided us a copy of the Explanation of Benefits We contract with various insurance plans and adhere to all contractual rates and discounts are not written off from our standard billing rates until the Explanation of Benefits is received from the patient's insurer and then the discount is immediately posted to the individual patient's account
