We are disappointed to hear about the experience that you had here in our office, as we do pride ourselves in the excellent customer service we aim to achieve with each patientPlease understand that insurance companies don't always pay 100% for servicesAs we try our best to work with your insurance so that the patient has the least out of pocket cost as possible, we are obligated to collect any co-payment that may applyWe apologize if this does not meet your expectationsIt seems that you have decided to try another dentist in which time we will release any information they may require as far as your records goPlease feel free to contact the office at any time to provide the new doctor's contact information so that we may send your records

As previously specified [redacted] did not provide an ID number or Social Security number over the phone stating that she did not have it on herShe did however provide her full name, Date of birth, and the name of her insurance carrier, which unfortunately is not enough to verify her insuranceAn ID or Social Security number is requiredWe have documented proof of this as wellWithout this information we cannot verify if anyone of our providers is in-network or out-of-network If the proper insurance information was provided and we were made aware that she wanted to see an in-network provider the necessary arrangements would’ve been made for herWe are here to help our patients and meet their needs or requests to the best of our abilitiesWe would have called her once we verified her insurance to properly accommodate her, if the information had been provided and we were aware that she wanted an in-network provider, as we do when any patient calls in requesting an in-network providerShe did call in stating that she was in pain and the scheduling coordinator was able to provide her with the earliest possible appointment so we could help her as soon as possibleWe did extend a helping hand to [redacted] since she was concerned about her crownsShe refused stating that she would prefer to go to another officeUnfortunately in this case there does seem to be a lack of communicationWe do hope that [redacted] is understanding on this matter as we cannot re-bill her insurance under an in-network provider that she did not see as that could be considered insurance fraudWe do hope that she does get her necessary treatment in a timely manner and we are also willing to comply with her new dental office with whatever records they may request

*** *** did call our office and schedule an appointment December *, for December ***, It was not specified that she wanted to see an in-network provider but rather if we participate with her insuranceThe patient did call in stating that she was in pain so we accommodated her with
the earliest possible appointmentDuring the phone call the Scheduling Coordinator did try to take down her insurance information, unfortunately the patient did not have the insurance information on her and did not feel comfortable providing her social security number over the phoneIf we would have known that she specifically asked for an in-network provider and we were able to verify her insurance before she came in for her appointment we would have made the necessary arrangements for herAs stated before we wanted to accommodate the patient with the earliest possible appointment since she mentioned she was in pain and luckily DrM*** was av ailable thenThe patient’s health was our number one priorityThe patient was also not over billed for the treatment that was rendered in our office, she did come in more than once to the practice and there was treatment renderedShe also spoke with one of the managers here who did try to alleviate her concerns and work with her so she could complete her additional treatmentThe patient stated that she would prefer to go to another office and wanted us to return the insurance payment and to bill once again in-networkIt was explained to the patient that that wouldn’t be possibleWe cannot bill under a provider that did not treat her because then that would be fraudulent in our partWe try to meet the needs of all of our patients to the best of our abilities, including *** ***Hopefully she is understanding of the situation and we hope this could settle the dispute

*** was seen in our office for her first appointment for gum disease on 5/**/She had extreme *** *** resulting from *** *** and stated that she “did not want to lose any teeth.” Her oral hygiene was/is very poorShe was advised as to the severe nature of her problemAt this
time, she also saw another dentist who advised her differentlyShe consented to treatment at this officeShe was treated, with follow-ups and maintenance total visitsOn 1/**/2014, her *** *** (her case) was noted to be non-existent and a notation made that “this case will fail if the patient does not maintain her oral health.” We have provided all the necessary treatment, as promised, but *** has not kept up with her periodontal maintenance appointments and does not maintain proper oral health habits at homeEven though ***’s initial treatment on 6/**/was a success, the treatment had to be redone on 1/**/due to inadequate o ral health maintenanceDrVictor T*** Steinway Family Dental Center

It has been brought to our attention that MrJahangir Khan is disputing the charges made by our officeAt this time the charges that were made are more than justifiable as the patient has been coming in for his dental treatment (attached you can find supporting documentation). It clearly
shows that the patient has already had his records taken (x-rays, impressions, intraoral pictures…etc.)On top of that he has had multiple exams with DrDayanayevTo the best of our knowledge, the patient does not have dental coverage which means he is responsible for all the stated services.According to the included Clinical Notes the patient was explained the treatment and he also approved to move forward with itDue to his decision the necessary trays were fabricated and ready for the patient to pick upThe patient then came into the office requesting a refund and was advised by the treatment coordinator that he spoke to, to come on a day when there was a manager in the office.The patient did not come in after thatA manager did attempt to contact him several times to settle this issue with no luck of getting in contact with him.The patient made payments for his treatmentThe first payment was for the amount of $2,made on 09/**/with the *** card ending in ***The second payment was also made 09/**/for the amount of $2,with the *** card ending in ***The third payment was then made 10/**/for the amount of $2,with the American Express card ending in ***.There were several attempts made in order to settle this dispute with the patient and to issue him a refundUnfortunately we had no such luck getting in contact with the patient in order to settle thisThere were messages left as wellIf any additional information is required please don’t hesitate to contact our office

I kept all requested appointments and when I went to the second dentist I wrote them a letter and victor asked me to return saying *** takes a while to regenerate but that I was doing a good job in cleaning he then said he would give me a periodontal cleaning for the $ usually charged by the dental hygienist when I went to make the next requested appointment for a cleaning I thought I would not have to pay because I had credit left from my insurance I was told he had charged me for the periodontal cleaning and I would have to pay this dental office cannot be trusted so I went to another dentist by my house who said I needed a *** *** and my teeth were still loose though victor said they were more stable I just retired and cannot afford game or thievery

apparently they are just going to keep saying the same thing  I brush after every meal religiously with an electric toothbrush and use an antiseptic mouthwash I stopped smoking more than 4 years ago which may have caused this problem but is no longer exasperating it maybe I should get in touch with the health department as some people suggested or find similar cases of trickery those would be the next steps

I specified that I wanted to see a in-network provider. If they said they did not have one, I would simply have called another office. It makes no sense to use an out-of-network provider. I gave the person on the phone the name of my insurance and my insurance id #. She was well aware that she was scheduling an appointment with a dentist that was out-of-network. When I got to the office on 12/*, I confirmed with staff that Dr. M[redacted] is in-network. They assured me he is. The Dr. himself told me he is in-netork. I called my insurer, [redacted] to go over fees. They gave me the procedure codes and what I should have been charged for the 2 visits. They charged me $1,324. instead of $677. My plan year maximum is $2,000 which is why I chose a participating PDP Provider. A fraud has been perpetrated. I confirmed with him that he is an in-network provider. My plan year maximum is $2,000. Why would I choose a dentist who isn't in-network? I did not have prior history with this office. It simply would not have made sense to do that. I never asked them to re-bill my insurance under an in-network provider. I asked them to do the right thing and reimburse my insurer for the amount they over billed. Over billing fraudulent, unethical and unprofessional.

To address these concerns, be advised that [redacted] was seen in our office 14 times for various forms of periodontal care. She has limited Oral Hygiene and was informed that without perfect home care, she will initiate further problems. She did have extensive periodontal work in our office on more than one occasion. Since her home care is very poor, it is to no surprise that she is not healing correctly. She went to another dentist who did not see the patient at her original state nor did the dentist evaluate her progress or her home care. He cannot make a judgement on the progress of the case.
