I am replying with regard to Revdex.com ID [redacted], patient of concern Mr. [redacted], [redacted], Janesville, WI 53546. I received the Revdex.com correspondence yesterday, [redacted]. I telephoned Mr. [redacted] and had a conversation. He requests a letter stating that he is disabled, in...
order to attain Social Security/Disability benefits. Mr. [redacted] was referred to the Social Security/Disability Administration to begin this process; notified that the SSA would then forward a medical questionnaire to this facility for completion. Mr. [redacted] confirmed his understanding of this process, and stated he did not understand he would need to go there for consideration of social security/disability. He stated he would move in this manner and stated the outcome was positive and no further action required on this agencies behalf. Further, Mr. [redacted] gave permission for me to follow up with your agency in this regard. Please also note, I am not aware of any prior requests for service of this manner from Mr. [redacted]. Review: no letter of surgery or description or explanation has been given to the limitation of my ability to perform my duties as an over the truck driver,since a medical card is needed per federal regulations,have not opptained one. need letter explaining surgery and results also fallow up reguest for medical condition.Desired Settlement: a letter sent explaining medical condition and reguirements to fallow from dr [redacted]/podiatry.
