2015 [redacted] signed up, on-line, and accepted our terms and agreements for membership. Ms. [redacted] came in on April 6, 2015 and filled out her cancelation form. She was given a copy of that form which stated her last draft would be May 1, 2015 (I have attached the registration form as well as the signed cancelation form).  On May 1, 2015 Ms. [redacted] called and was very upset that she had drafted that day.
