purchase of Prevident toothpasteAccording to Internal Revenue Services (IRS) requirements, reimbursement of specialized toothpaste is conditionally eligible for reimbursement based on medical necessity and should only be reimbursed for the amount over the cost of non-specialized toothpaste.Initially the letter of medical necessity was not submitted with the request for reimbursement and once submitted, was not written to cover the date of the purchase as required by the IRS and noted on the BenefitHelp Solution Letter of Medical Necessity formBenefitHelp Solutions has since received all required documentation.This matter was resolved on December 20, On December 21, 2016, Ms*** was issued a direct deposit in the amount of $into the checking account number on file, provided by Ms***.BenefitHelp Solutions considers this matter closed at this time.Thank you,

Thank you for affording BenefitHelp Solutions (“BHS”) an opportunity to respond to the complaint filed by [redacted]. After careful review of the facts relating to this matter, I found the following.During the 2016 plan year, Ms. [redacted] used a Benefit MasterCard for reimbursement of expenses...
from her health FSA. Certain Benefit MasterCard transactions are automatically substantiated at the point of sale, while all other transactions require that additional third-party documentation be submitted. It seems that an issue arose due to the timing of the use of the Benefit MasterCard and the receipt of required substantiating documents for several card transactions. This timing issue resulted in Ms. [redacted]’s Benefit MasterCard being temporarily turned off. Upon receipt of all outstanding documentation, BHS reactivated Ms. [redacted]’s card. At Ms. [redacted]’s request a new Benefit MasterCard has been issued.In order for an eligible medical care expense to be reimbursed from a health FSA, both a statement from the participant and supporting documentation from a third party must be provided. For expenses incurred in 2017, Ms. [redacted] manually submitted several claims for reimbursement without a signed claim form. Reimbursement on these claims was pended until a signed claim form was received. At this time, all pended claims for the 2017 plan year in which BHS has received the necessary documentation have been reprocessed.Unfortunately, the communications provided to Ms. [redacted] may have resulted in confusion regarding what information was needed to resolve any outstanding transactions.
