*** *** spouse, *** *** is a participant in an employer-sponsored *** *** *** *** *** plan which allows the employer to contribute money to a trust on behalf of its employees to be used to help pay for employees’ eligible medical expenses through a Health Reimbursement Arrangement (HRA), administered at BPAS, IncThe employer elected to offer its participants a debit card as an added benefit to pay eligible medical expenses upfront, thus eliminating the requirement for out of pocket payments at the time an expense is incurredThe IRS requires claim substantiation on all HRA expenses, including debit card expensesHowever, if the debit card payment is equal to the Employer's single co-pay or up to a multiple of times a co-pay for services rendered, there is no need to submit a paper claim following the card swipeThe IRS has issued guidance (IRS rules gover ning the substantiation requirements are further detailed in Revenue Ruling *** and Notice ***) that allows for the match up of such expenses as sufficient substantiationFor all other expenses (deductibles, co-insurance, discount vision plans, over the counter items, etc), the participant will receive a receipt requirement notification from our claims processing department requesting the receipt to substantiate the expenseA receipt is explained in this notification as an itemized statement from the provider that includes the date of service and type of service rendered, or date of service and name of the prescription or over-the-counter items purchasedThe expense will remain in a “pending” status while awaiting substantiationBPAS, Incwill send reminder notifications to the participant over a day period in an effort to collect this outstanding substantiationIf the expense goes unsubstantiated over days, the debit card will be tempor arily suspended until the outstanding item has been resolvedParticipants are not denied access to the funds; reimbursement is always an optionDebit card suspension only stops the card from being used for future charges; it does not stop the ability to be reimbursed from previous claimsIt is a card suspension, not an account suspension*** *** claim that caused the debit card suspension was for services provided on 2/**/The vendor was paid for those services on 2/**/*** *** stated that she requested the receipt for this service be faxed to BPAS directly from the service provider; however BPAS never received any documentation from the service providerThe participant was sent communications on 2/**, 3/**, 4/and 5/** requesting documentationWith the exception of final communication on 5/** stating the debit card was placed on suspension, the card was still usableAll funds in *** *** account were still accessible to the participant upon submission of a claim reimbursement request for out of pocket expensesTo date, the claim for services provided on 2/**/has been substantiated; *** *** submitted the necessary receipt by fax on 6/**/The claim has been marked as paid and the temporary suspension was lifted 6/**/The card is currently activeWhile we regret any inconvenience that may have been experienced through this situation, BPAS handled the matter in accordance with IRS rules and our documented policies, which are published in our product for the benefit of plan sponsors and participantsThank you for allowing us to clarify this matter for the benefit of all involvedBPAS, Inc
