August 4, 2016In response to this complaint, this patient had medical services on 08/03/She presented her insurance cards and paid her co-payThe insurance was billed the balance but rejected the claim and denied paymentThe patient was advised in writing that she was responsible for all charges if her insurance did not cover themI have enclosed a copy of the signed authorizationThe confidential info is blocked out.Pt's insurance frequently does not cover some medical procedures or they have deductiblesIf the patient feels it should have been covered she should have had that discussion with her insurance carrier.The patient was sent multiple bills at which time a solution or reduction of the charges could have been madeThe company that does our billing forwards these accounts to collections if they are non responsive.If the Patient had responded two years ago after the charges were denied (notice was sent to her by the insurance carrier and us) this might have been resolved quicklyThe patient still has this outstanding bill for services rendered and received.Thank You Berger & Benjamin LLP

I am rejecting this response because:The service date of 08/03/14 was paid in full with cash that the office should have in a receipt book as a carbon copy. The services being questioned were in the year of 2013. This office is providing information that goes against HIPPA because, you have no signed record release form to even show my signature for a service that is not in question. The office's practices are questionable completely and I have not authorized this information to be released for any dates of service. It is clear this office has no clue as of to what they are doing. Now I will be seeking legal actions for invasion of privacy.
