September 15, 2016Dear [redacted]Best Practices, Inc. has received the attached letter of complaint regarding services provided to [redacted] at [redacted] Hospital December 05, 2013 for review and follow up.I understand that [redacted] believes the claim was paid by her insurance carrier...

and the account should be satisfied.In this letter, we will address the status of the account to date.I understand that [redacted]'s insurance carrier [redacted] stated payment was made to our office directly, however, On August 5, 2016, the insurance representative contacted our office advising that the zip code that was on file was incorrect. This issue has since been corrected and payment is forthcoming.Once payment has been made on the account, [redacted] will receive a revised billing statement for her records.We do hope this letter provides some resolution to the matter and we appreciate the time you have taken to address the patient's concern over the emergency room physician's billing.Very Truly Yours,Aqueelah J.
