Review: I am responsible for paying the bills at home and when I received a bill for lab services from Beth Israel for my husband [redacted], I call Beth Israel and requested a deferred payment plan which was offered. After going over the payments I realized that I had overpaid Beth Israel Laboratory $100.00 which was billed to my husband - dated 8.**.2013 and was due on 9.**.2013. I called the billing off shortly after but they had not received the check. On 10-**-13, I called and spoke with [redacted]. She admitted the check was received but it was not applied and promised to find out why. On 10-**-2013, I spoke with [redacted] but still couldn't get an answer regarding the overpayment. On 12-*-2013, I called again and spoke with [redacted]. I was informed that there was an overpayment of $100.00 and promised to start a refund process which she stated would take up to six weeks. To-date I have not received the refund check.Desired Settlement: Send a refund check asap.

Review: After receiving care from an affiliated physician for a yearly physical, blood work was sent from from my doctor's office to the Beth Israel Laboratory for testing. Typical with this transaction, the doctors office correctly provided the lab with my insurance information (Which I'd confirmed would cover the costs of the lab expenses). Upon receiving the blood work and insurance information BIL incorrectly matched my account with an insurance plan and number that was not mine. The payment window for a lab to receive reimbursement from my healthcare provider was 180 days. On the 181 day, without having once requested my health insurance information (since the information they incorrectly were trying to use was denied), BIL immediately sent me a bill for the full costs of the lab work. These bills did not include any information about why they were billing me directly or that at any point in the process they'd waited six months to identify they had an issue with my insurance). As it stands, BML has informed me that I am still responsible for the costs of the lab fees. (administratively if they had identified the issue and contact me in January, not the day after the 180 window closed, there would be no issue for them to bill my insurance and they'd have no reason to try and recuperate their loss directly from me). As it stands I can confirm:

-the the doctors office both collected and billed my insurance for the services they rendered.

-The doctors office provided the correct insurance information to BIL

-BIL's first attempt to contact me was exactly 6 months and 1 day after my visit to the doctor

-BIL had the incorrect insurance information on file for my account and never attempted to rectify the information before attempting to bill me directlyDesired Settlement: Since services rendered were completely covered under my insurance and I, in good faith, provided all information necessary to process the information in a timely manner, i'd like the bill to be removed from my account and all information reported to collections agencies to be removed.

Review: For lab services rendered (account #[redacted]) on 1/**/14 my insurance company originally declined to cover the claim for $345 and I paid the amount in full by personal check on 4/*/14. On 2/**/15, the insurance company adjusted the original claim and paid BIMC $168.40 and my responsibility under my insurance was reduced to $142.01 resulting in a refund owed to me by BIMC for $202.99. On March **, 2015, I emailed them and was told " Upon review of your account, I do see that you have a refund of $202.99 due to you. I have started the refund process. I do want to advise, the refund process does take some time. But, you will be receiving a check in the mail for the said amount.". On May **, 2015, I contacted them again and was told to call [redacted] at the main office for refund phone# [redacted] I called the main office and my call was bounced around to a few different people. I finally spoke with someone who informed me that the refund would be mailed to me. It is now June **, 2015 and I still have not received the refund.Desired Settlement: I would like my refund check. The lab expects to get paid right away when they invoice you so why will they not issue a refund. I have all the paperwork supporting this claim.
