This letter is in answer to patient *** *** complaint.*** ***She first came to our office on 4/13/with a right ankle fracturePut in aboot and told to return in weeksCollected a $co-pay and a claimof $was filed on 4/13/UHC paid $0, adjusted $34.84, leaving
abalance of $patient responsibility because there was no referraltherefore goes to her deductible(See attached) Four statements wereissued to Ms*** in a four month span, never hearing one word fromher, also leaving phone messages regarding her balance dueShe alsocame in on 5/11/for a follvisit and xraysCollected a $copayand a claim for $was filed on 8/11/That claim was deniedfor timely filing (See attached) and written off and Ms*** was notcharged.This was sent to collection on 2/17/after months of never hearingfrom her and the collection agency finally got her attention!!! If she wouldhave reached out to us this could have all been prevented, but bottom lineshe still owes $
