Customer called on 10/10/17 and left a voicemail to inquire about his policy. Our agent then spoke to his wife and advised her that the policy had been cancelled by insured request in May of 2017. She replied that they had not requested cancellation. Our agent advised her that a phone request to...
cancel was documented and that the party who called asked that the form be mailed to a different address located in [redacted] Ohio. Customer also denied requesting the address change. However, we did receive the signed cancellation request and proceeded with canceling the policy. Our agent told his wife that the form was signed and dated by the husband. We have since discovered that the form was signed by another insured with the same name.The unearned premium from cancellation of the policy was returned to [redacted] Insurance as undeliverable. At that time, our agent called and left a voicemail for the insured. No response was received. The customer was on a monthly pay plan for the policy in question. Apparently no payment was made for June, July, August or September since this was just brought to our attention on October 10. Immediately upon learning of the issue, we rewrote the customer's coverage and requested that [redacted] send the return premium due him from the cancellation to his current address.Our agent followed proper procedure for cancellation of a policy. She requested and received a signed cancellation request form. It is important to note that even if this had not happened, the policy would have canceled in July for non-payment of premium since no payments were made after May. The insured never questioned anything until October 10.No financial loss was suffered by the customer during the lapse in coverage.We apologize for the Inconvenience to this insured and we have corrected this situation as soon as we were made aware of the issue.
