November 15, 2017Dear [redacted],I reviewed the aforementioned 5 page complaint as it pertains to Blaum Breast Care Specialist filed on 11/08/17 by [redacted] Plains, PA [redacted]. The complaint afforded [redacted] theplatform to air her grievances about our office protocol concerning her co-insurance which she owed for services rendered on 09/27/17. The complaint stated that [redacted] feels she was overcharged by  $12.63 however, the record of the patient's chart does reflect the same sentiment.Attached please find an inquiry from [redacted] for Eligibility and Benefits for [redacted] dated 09/15/17researched by my staff. Inquiry concluded the patient owes 15% Co-Insurance per visit. A secondinquiry by my staff conducted on 09/21/17 via. Phone call to [redacted] rep. [redacted] confirmed thatpatient owed $112.00 for CPT Code [redacted]. which is the procedure performed. A second inquiry is thestandard practice prior to biopsies to see if deductibles and or patient responsibility changes from initial office visit to day of biopsy. Most times it does not change however, we like to err on the side of the patient. [redacted] signed the consent dated 09/19/17 making her aware of her obligation prior to the procedure on 09/27/17.Furthermore, please find an Allowance inquiry from [redacted] dated today which my Third Party billingcompany sent to me further supporting the policies of this office and our billing procedures. The[redacted] Allowance Inquiry states that the CPT [redacted] which [redacted] had allows $ 753.50.if you apply her 15% Co-insurance, the net result is $113.02. The aforementioned conversation on09/21/17 with [redacted] the [redacted] rep said $112.00. This leaves a balance owed to us of $1.02.Whether l bill [redacted] for this amount or not is to be determined.We do not choose a patient's insurance. We can only go by the information given to us or attainedthrough the process of discovery. Clearly [redacted] is not proficient on the basic standards of practice for business as it pertains to Accounts Payable and Accounts Receivable and any refunds that may arise.  Thirty (30) to Sixty (60) day turn around times more than meets these voluntary accounting guidelines.  Blaum Breast Care Specialist, LLC is no exception to this standard.The Account Ledger for [redacted] received in my office on 11/07/17 in our October Month End packetshowed a Co-insurance payment of $99.37 on 10/11/17. [redacted] was charged $112.00 per herinsurance inquiry resulting in a credit of $12.63.My billing company which is not in house told [redacted] that her refund was in the system and it would be sent to my office and paid. I got a message on 11/8/17 that a very irate woman demanded her $12 dollars and threatened to report Dr. Blaum to the Revdex.com. in turn, I phoned the patient and she did not answer so I left a message and said the refund would be processed on Friday the 10th of November and that she should receive it Monday or Tuesday the latest.She did not have the courtesy to return my call. I guess this complaint is her rendition of courtesy to this office and Dr. Blaum for helping her and giving her the good news of NOT having Breast Cancer.This job is tough enough on its merits. When I have to spend half of my day to clear up what should have been cut and dry had the insured [redacted] read her insurance policy, which SHE SELECTED, andunderstood the ramifications of the process in question.Regard ,  Rebecca B.
