Dear...

Sir or Madam:We have received the complaint filed by [redacted] Prior to receiving this complaint we received a letter of complaint from the Revdex.com. The contract [redacted] entered into was a bank draft contract. [redacted] bank account was to be drafted the 1st of each month in the amount of $52.00. The original contract was dated June 22, 2017. The first dues posting was July 1, 2017. As you will find in the paperwork the account was active upon completion and the family began utilizing the facility on June 26, 2017. On September 6, 2017[redacted] came in to remove a minor child on her membership and add another. Upon review of the new application in back office, we found there was missing information for the minor child and proceeded to contact [redacted]. The mobile number listed was a non-working number. Second line of defense is to utilize the work number. We proceeded to contact Ms [redacted] through the number she provided. We asked to speak with [redacted] to explain the situation. We were placed on line with [redacted]. After a description in detail of what we needed to proceed, we were disconnected and received no return call. At that point we placed the membership in freeze status until [redacted]l could appear in person to resolve the situation. [redacted] now states that she suspects there have to be two [redacted] employed at the [redacted]. And she never spoke with us. Later that night she contacted the center and was vey abrasive stating her child was turned away. She was given instruction to appear in person to resolve the issue since our attempts to contact her through information she provided were unsuccessful. She appeared in person on September 8, 2017and was asked if she would like to provide the missing information on residency for the minor child. She stated that only two of the children were her children legally and could not provide proof of residency for the additional child. She was given the option to update the information and continue the contract. She stated she did not wish to continue the membership. When a member chooses to not continue in contract there is an early termination fee imposed, clearly stated in contract as well as proof of residency clause. The $100 early termination fee was not imposed in this instance as [redacted] was not happy with services. She was drafted July, August, and September 1st 2017, each draft for $52.00. The services were utilized through dates stated. A member check in reports was printed and enclosed to show utilization. A copy of the termination form she signed and dated is enclosed. Reason for termination she states that GBCC terminated the contract. We only placed it in freeze mode until issue could be resolved. She was given the option to update information and remain in the contract. [redacted]sked that her original voided check be returned that she used to start the draft initially. We pulled the check from the files and returned it immediately. Once she had the check in hand she refused further services.Kind Regards,[redacted]
