Hi ***: In response to the complaint, the policy that *** *** purchased thru our company does not cover interscholastic sports. The injury that was presented to us was a ice hockey injury at *** ** on 1/27/13. So we were unable to pay the claims under that
policy. However, the school does provide compulsory insurance that covers interscholastic sports. Unfortunately, the administrative office at the school would not provide a claim form because *** *** didn't contact them within one year of the accident. We have been in contact with the school and a recent email from *** *** provided proof to the school that she did in fact email the school back in January of 2013. The school has advised us that they will be forwarding to us a claim form and we will do our best to process the bills as long as we have all the necessary information. We will be in contact with *** *** should we need any additional information. The compulsory policy thru the school requires a signed claim form from the school validating the accident. This also authorizes us to pay a claim on their policy. Without the claim form BMI Benefits cannot process the claim, we were trying to resolve this with the school and *** *** when we received your complaint. Review: My daughter was injured at her day care and needed an ER visit. My day care has this insurance to cover outside costs associated with injuries that happen there. Her injury was in August, and this is December. All of the information was submitted in a timely manner, but the bill was continuously not paid. I got the day care's administration involved and we were assured the bill would be paid. Now it is going to collections and affecting my credit. This company has done NOTHING; they are reprehensible. My child was hurt and I have gotten nothing but stress and now my credit is being affected. I cannot express my disgust with their practices.Desired Settlement: It's simple: Pay her ER bill

We spoke to the complainant and informed her that our check #[redacted] for the balance due of $183.54 was issued on 12/16/15. We are a secondary payer for student accident insurance which requires us to get certain information from the primary carrier before we can process the claim. There was some confusion as to what we needed to process the claim. The matter has been resolved.

Review: I secured insurance through [redacted] Insurance (BMI) for the 2012-2013 school year. My daughter was injured during ice hockey practice in January 2013. I send [redacted] a letter and the hospital bill in March 2013. I started receiving collection calls in November 2013 for non-payment of the bill. I contacted BMI in Nov 2013 and they acknowledged that I had a policy with them but that they couldn't find my letter. They also advised that I needed the school to submit a form. I contacted the school and was advised that they didn't have any forms. BMI said they couldn't send me the form and I explained that I was at a loss why the school needed to send in a form when I had insurance directly with them. It wasn't until February 2014 that BMI finally sent me the form for the school to complete and that they would process the payment at that time. The bill has not yet been paid by BMI and I have had to start payment of the $1,337 to stop collection action. I am still at a loss as to why Stamford Public Schools are involved since I had private insurance and in order for my daughter to participate in sports I had to sign a waive of liability for the school. I would like BMI to step up to their obligation and pay this $1,337 bill for which I had insurance through them. No one is disputing whether I had insurance with BMI not ia anyone disputing that my daughter was a student and that she was participating in ice hockey. I am at a loss why this payment has not yet been made by BMI.Desired Settlement: I would like BMI to honor their obligation under my insurance policy and send the payment of $1,337 directly to me since I have had to start repayment of this bill to stop collection action.
