seemed to feel fine, the patient wished to have another set madeThese were delivered in May The patient was seen for follvisits, at which time she stated things felt fineThe last visit with the patient was in July The patient was told to come back for any additional adjustments she feels are necessaryPatient never called for additional visitsThe patient has paid $out of pocket towards this expense, and the insurance portion which was paid was stated on the statements which she receivedOur office has since contacted the insurance company about resolving this issue, and they informed us that the patient contacted them in January and was told to contact our officeThe patient has never contacted our office about resolving any issueOur office, therefore, will be contacting the patient in order to help her resolve any issues
