This patient is correct that I contacted him on 2-17-to tell him that our laboratory confirmed that although two ordered tests were not performed, several others were performed and completed In addition, we confirmed that the tests that were billed to the patient’s insurance were indeed those that were completed The issue that Mr [redacted] filed with your office indicates that he is concerned about the accuracy of the information we provided him in February 2017, so I contacted our Laboratory manager once again today and confirmed the same information she gave me in February I then contacted the patient’s physician office and spoke with a member of her staff who is familiar with the patient She reviewed his medical record and confirmed that the doctor went over the results of these tests during the patient’s 11-15-office visit The physician’s office offered a couple of options to assist the patient with learning more about the lab tests that were done: The patient could make an appointment to meet with the physician again to go over his test results, or The patient could contact the Beloit Health System Medical Records Department to authorize the release of his lab test results and medical record to him I called the patient today and gave him the suggestions above that were offered by his doctor’s office If you have any additional questions or concerns about this case, please feel free to contact my office at your convenience Thank you for your kind consideration of this matter [redacted] , FHFMABeloit Health System, [email protected]

I have been trying to reach the Dr.'s office, but am unable to leave a message.I went to the Dr.'s office on Friday afternoon, but they were not working?AGAIN, I had draws for testing, but was given results for only The 5th test was lost.However, I was billed for all How am I supposed to get this matter resolved when I cannot reach anyone at the hospital.Please re-open this complaint.Thank you

Pursuant to our previous responses to the Revdex.com as well as Mr***, we encourage the patient to contact the Beloit Health System Medical Records Department and request a copy of the results of all tests. Mr*** may also contact the Beloit Health System Laboratory Department and request a copy of the results of the tests. He should present to either department in person so he can provide photo ID as confirmation that he is the patient. Our office confirmed, for the fourth time today, that all tests billed to his insurance company were indeed completed. As we stated in our previous response, Mr*** is welcome to make an appointment with his physician to go over the test results as well. However, he must make an appointment to see the physician so as to assure that the physician will be available when he arrives. Please contact my office directly if you have any further questions.
