Review: Sometime in July 2013 my wife, [redacted] attended the office of [redacted]. [redacted] DDS at the Bozeman Dental Associates [redacted]In late September 2013, Dr. [redacted] submitted a bill for $10.30 which was apparently an insurance co-payment for his services some months before.I promptly paid D. [redacted]'s bill of $10.30 by direct payment by check from my bank, Bank of Bozeman. The amount paid was $10.30, the check number was [redacted], the date paid was October 3, 2013, the account number is [redacted].On November 9, 2013, Dr. [redacted] again billed me for $10.30, his bill had the notation, "PAST DUE".Attempts to contact Dr. [redacted] at the Bozeman Dental Associates using the email address shown on his web site were unsuccessful as that email address did not work. Dr. [redacted] does not have a fax number or the fax number is not apparent.I fear the usual outcome -- being forced to deal with some remote collection agency, periodic harassing telephone calls and the usual threats and additional efforts on my part to resolve problems with my credit rating which is excellent.Desired Settlement: I need documentation showing the dates of service, patient name, code, description of service, account number, amount of payment, date the payment was received.I request that this statement show: PAID IN FULLThe statement will be dated and identify the payee.Should Bozeman Dental Associates have lost payment records for this account or applied the payment to the wrong account this will be of no concern for me.

In regards to case #[redacted],
customer [redacted], we have resolved the issue. [redacted] contacted our
office for the first time on 11/11/13 stating he had made a payment: in the amount of $10.30
that was not applied to [redacted]'s account. After reviewing his complaint,
that same day, we found that the check was applied to the wrong account. This
was an honest error, we immediately applied the $10.30 check to [redacted]'s account
on 11/11/13. Her account is now at a zero balance.
