I paid $725.00 on 7/2/2018 for an interim partial dr (***) before submitted to insurance. Brandywine Smile Center sent it to my insurance company (***)June 26, 2019. The explanation of benefits said "customer's responsibility $322.00". I contacted them 7/18/19 by phone and email asking to resolve it. I sent letter August 12, 2019 trying to resolve it. When I didn't hear anything yet, I sent a certified letter September 20th.
I have all the paper work.
