I believe my letters to Ms [redacted] explain our position. She provided an insurance card at the hospital that was expired. Multiple letters were sent to Ms Schenback asking for her current insurance card. Initially, March 2015, Ms Schenback...

claimed she never received the letters because we were "sending them to the wrong address". Then, in June 2015, Ms [redacted] sent a text to my biller that was a picture of one of the letters that she denied receiving and stated she had already sent me the correct insurance information "multiple times". To date, the current insurance card has not been received.
When billing insurance for surgical assistant fees, an "AS" or "80" modifier is used along with the surgeon's codes and charges to signal the insurance company to pay the surgical assistant at the surgical assistant fee. As you can see from the attached epson96 and epson98 files, those modifiers were included. She is welcome to call her insurance carrier or any other insurance carrier, to verify this method of billing. My biller admitted she initially sent the wrong amount to the collections agency, but called two days later to correct her mistake. By this time, the collections company had already mailed Ms [redacted] a letter. Since we made the error and Ms. [redacted] claimed she never received any of the previous bills (despite the text she sent of one of the bills she claimed she never received) Ms [redacted] was offered a second chance to pay her bill with the correct amount owed.
Due to Ms. [redacted] unwillingness to provide us with a copy of her insurance card, we can no longer bill her insurance company as we are out of timely filing. Timely filing is 90 days to 6 months to file a claim with the insurance company after the procedure. In my letter to Ms [redacted], we encouraged Ms [redacted] to contact her insurance carrier and see if they will reimburse her for cost associated with the procedure. All codes and charges have been given to Ms [redacted].
