Please be advised that *** called our office on January 25, 2017, after receiving our post card in the
mailShe stated she was interested in sedation, and would like to know vvhat treatment she needed
Upon arrival for her appointment on January 31, 2017, *** did not present any of our coupon offers on the postcardWe proceeded to take the necessary x-rays and tests to complete our exam and present *** with the treatment plan she requested*** was presented a sedation treatment plan, signed the treatment plan, and completed all paperwork and consent forms required for the treatment (see attached primary treatment plan and sedation paperwork)Her vitals were taken, and medications Diazepam and Triazolam were dispensed for the sedation treatment*** then spoke to treatment coordinator to go over financials of prepayment and to make an appointmentHer appointment options were either February or February 24, but she stated she would call us back the next day because she needed to request off workShe asked us to hold the two dates for her appointment, and said she needed to transfer money front her Credit Union (see attached progress notes)_ We don't usually dispense medication unless the sedation appointment is made and prepaid in full, but *** reassured us the money and appointment would be confirmed the next day, February 1, *** did ask if she brought in the sedation coupon if we would honor it and eredit the sedation portion of her treatment plan, which I did agree to and this is noted on her attached primary treatment plan
We called and left messages several times after not hearing from her (see attached progress notes)Since we did not bear from the patient, we cancelled the hold on the appointment datesOur office called *** on February 9, 2017, and February 13, 2017, to ask if she was still interested in the sedation appointment, but she never returned our callsStatements were sent to the patient on February 1, March 1, March 29, April 26, May 3, and e-ntailed to her on May (see attached account statement), On May 8, a phone call was made to *** regarding her account balance and a voicemail was left explaining that a payment needed to be made to avoid it being turned over to collections*** did sign our office policy on January 31, 2017, which states, "If a payment is not received by the statement due date, your account will be considered past dueWe reserve the right to charge your account a rate of 1.5% or a minimum of *** monthlyUtile balance continues to go unpaid, the account will be turned over to an agency resulting in the patient's responsibility for all attorney/collection/court fees that this office incurs while attempting to collect the debt" On May 12, 2017, *** called the office regarding the statement she received, and my office manager, *** ***, explained to her that the services we provided to her on January were not part of any "free" offerThe only free offer we advertise
is for an implant consultation, which was not part of her treatment plan we discussed*** became very upset, stated she was going to give a bad review, report me for fraudulent advertising, and hung up the phoneShe then proceeded to use social media to ruin my reputation with allegations
I have forwarded all patient records, consents, treatment plans, financial arrangements, and progress notes to you per the Office of the Attorney General request received

Creating Smiles response is totally incorrect and a blatant lieUpon arrival not only did I present the coupon but I gave it to the receptionist when I arrived and sign inThe receptionist told me the coupon that I choose to use would be my choice and I should choose after the evaluation
I received the treatment plan paperwork and was told I should read and sign it, in which I did, BUT it was prior to me receiving the actual price for the treatmentOnce I was presented with the price I stated "I will have to think about it and get back to you guys"I never asked for dates to be held for meThe scheduler listed some possible dates that were open if I decided to go with the treatmentI never stated ANYTHING regarding needing to request time away from work, because after seeing the price I knew I would be seeking a second opinion before making any decisions and I'm glad that I didDr*** treatment cost was $15,for two Root Canals, cleaning and a few other minor thingsI went to another doctor and received everything for less than $2K
In regards to them calling me, yes they called to do a follow up and attempt to convince me to get the treatment that Dr*** offeredNever once in the prior calls did they mention anything about a pending bill or charges
Lastly, in regards to them mailing me statements, this is so untrue as well
On May 12th, when I received the phone call regarding a bill and charges, I immediately called back and inquired about this "bill"Initially, I was told it was probably a mistake and they would investigate itTwo days later, I received a call stating the bill is "As Stand, and you owe *** Additionally, up until this point and even as of today I have yet to receive any bill in the mailThe only bill that I have received up until now is the one that I requested to be emailed to meWhen I questioned the office staff about why was this the first time of me learning about any charges they stated that they had been sending the bill to the wrong addressI find that very hard to believeNot only does my Drivers License have my correct address, but so does all the paperwork that I filled out
I'm not sure of the type of practice that Dr*** is running, but honesty and integrity goes a long way and for a dental practice to try to scam a customer, it says a lot for that companyIn Dr*** response, he clearly stated when I initially called to schedule an appointment I referenced his coupon mailerA coupon mailer that had currently been disseminated by his companyThe same coupon mailer that I gave to the receptionist when I checked inThe coupon that they failed to apply to this "bill"
I am a Registered Nurse and in my practice I'm faced with ethical decisions everydayThe minute I saw the cost of the treatment I shook my head in disbelief and told the staff that I would think about itWho would make a decision within ten minutes for a service that would cost $15KThat's a major decision
I questioned the scheduler about the four pills that they gave me and she stated it was no big deal and now I'm being charged $for a consultation that I was initially told was freeThis is unethical and I'm now going to report this company to the news channels and I'm going to continue reporting them until someones does something about this
