Kristin the hygienist during scaling noticed the DB cusp of #was missing, she asked pt about it, she said it has been gone for a long time and that she doesn`t want to do anything about it [redacted] took new IO photos today so we can watch it DrD [redacted] had an opportunity to talk to [redacted] and then explained to her that tooth #had a very large amalgam filling and that there was obviously a fracture in that cusp that was not visible clinically, or radio-graphically I explained to her that good, solid tooth structure and fillings will not fracture during a routine SRP and that if left alone the tooth would have eventually fractured on biting, grinding, etc Pt understood and said she was happy Pt wanted to proceed with the crown as she is going to Red Lodge for a painting workshop, and did not want the tooth to fracture any further I told her that she would be in a temporary crown for weeks, and there was a small chance there could be problems with the temp, especially if she ate anything sticky, or hard on the side We will proceed with the crown todayBrewer Dental Center did provide a credit for a buildup on tooth #to help support the new crown

[redacted] and then explained to her that tooth #14 had a very large amalgam filling and that there was obviously a fracture in that cusp that was not visible clinically, or radio-graphically.  I explained to her that good, solid tooth structure and fillings will not fracture during a routine SRP and that if left alone the tooth would have eventually fractured on biting, grinding, etc.  Pt understood and said she was happy.  Pt wanted to proceed with the crown as she is going to Red Lodge for a painting workshop, and did not want the tooth to fracture any further.  I told her that she would be in a temporary crown for 2 weeks, and there was a small chance there could be problems with the temp, especially if she ate anything sticky, or hard on the side.  We will proceed with the crown today. Brewer Dental Center did provide a credit for a buildup on tooth #14 to help support the new crown.
