have a cleaning done for four and a half years and he had heavy buildup and subgingival calculus and stains. He had bone loss interproximally. He was explained that before we do the teeth whitening he needs to get his gums and teeth clean and healthy. With his condition of his gums and teeth whitening is not give him any results as he has heavy buildup and plaque. But he insisted that he does not want any deep cleaning and he only wants polish and teeth whitening. He has further explained that he will get teeth whitening for sure but after his gums and teeth are clean to get the result for whitening. I told him that just basic cleaning is not enough for the health of his gums. He got upset and started yelling at me and the staff. it is against my morals and ethics to do the wrong treatment on the patient that he does not need. I went to school for 10 years to diagnose what treatment the patient needs and I am authorized to diagnose the treatments or the patient. I am here to make sure that before I do elective treatment like teeth whitening that the patient's gums and teeth are healthy. I don't want to charge the patient the treatment that he don't need. Patient cannot diagnose his own treatment; he did not go to school to be a Dentist. I went to school to diagnose the treatment for patients. So, if he doesn't want a deep cleaning, we did not want to do basic cleaning because that is what he doesn't need. He can go to another office to do the treatment he wants and if that Doctor doesn't have any problem in doing that but in my office I will do the treatment that is needed for him. His insurance was billed only for x-rays and exam.His wife [redacted] (chart #5495) was seen in our office as a new patient on 12/16/2016. Full mouth x-rays were done and she needed treatment. She has not been to the dentist for the last 5 years. She had heavy buildup and plaque and heavy subgingival calculus with heavy stains. The chart notes from hygienist Mrs. [redacted] are enclosed along with the copy of the x-rays that show the subgingival calculus. She was explained the treatment and she already knew that she needed the deep cleaning, so she signed the consent and started one side of deep cleaning. She was scheduled with the hygienist for 1 hour and hygienist was able to clean only one quadrant, due to heavy plaque and buildup. She was scheduled for next week to do another quadrant, due to heavy plaque and buildup. She was scheduled for next week to do another quadrant cleaning. The husband called and cancelled her appointment and told us that she will not be coming back. Her insurance was billed for x-rays, exam and only one quadrant of cleaning.
