We did not follow our internal documentation policy regarding this patient, there are minimal notes to verify we had contact with the patient or his family.  In light of this evidence we are adjusting his account and paying back the money to his insurance company.   We have retrained our staff on our documentation policy.  We apologize for causing [redacted] frustration he will receive no more bills from Burman’s Medical Supplies.
