l am in receipt of your letter dated August 18, in regards to Mr [redacted] ’s account I have reviewed his account and Mr [redacted] does owe $which represents his coinsurance amount from his [redacted] Policy I contacted [redacted] and spoke to Tracy, Reference number [redacted] she reviewed his claim and found that there was no second billing to [redacted] from Butler Memorial HospitalThe reason for the amount to increase was [redacted] did a review of his claim and found that he did not have Medicare Part B coverage until July 1, which caused [redacted] to penalize the patient for a higher out of pocket expense [redacted] retracted their original payment and paid the claim at a lower amount causing Mr [redacted] ’s coinsurance to increase Listed below is series of events that occurred on Mr [redacted] ’s account • [redacted] was billed on 8/1/ • [redacted] denied claim on 9/11/for coordination of benefits • Bill was sent to patient 11/1/ • Mr [redacted] called Butler Memorial Hospital on 12/2/we told patient to call [redacted] to discuss why claim was denied • On 12/11/Mr [redacted] contacted our office to ~et us know he received a letter from [redacted] stating the claim was processed for payment • On 12/11/we did received payment from [redacted] with patient owing $coinsurance • Mr [redacted] began making monthly payments of $starting in January to Butler Memorial HospitalTotal amount paid by Mr [redacted] so far is $ • On 7/16/ [redacted] retracted their original payment and repaid Butler Memorial Hospital at a lower rate stating now the patient owes $coinsurance

I spoke to Mr*** *** *** today in regards to his balance due at Butler Memorial Hospital. I told Mr*** that we will honor his request that he owes the original copayment amount of $Mr*** has already made payments totaling $to Butler Memorial Hospital, his balance due is now $140.29. Mr*** was satisfied with the outcome of the callI told him that I would be emailing you today on our phone call If there is anything else further I need to do to close this case please let me know

I have been dumbfounded by the lack of clear information about this matterThe details the Butler Memorial Hospital gave only came to light after involving a third party, i.eyourselves--a step that I should never have had to takePreviously, Butler Hospital blamed *** *** and vice versaI wonder how much longer I would have been asking for the truth had I not consulted your company, and for that I do thank you
With regard to their response, Butler Memorial Hospital seems to have filed incorrect information from the original service/billing dateI never stated at the time of service that I had Medicare Part BIn fact, my original insurance card from that time (which I have attached) clearly states that I only had the Part A benefitAny assumption that I had Medicare Part B and should have been billed that way was entirely the fault of Butler Memorial Hospital and not myselfHence, they should resolve the error instead of doing what amounts to penalizing me for their mistake

I have reviewed his account and Mr. [redacted] does owe $550.38 which represents his coinsurance amount
from his [redacted] Policy.
I contacted [redacted] and...

spoke to Tracy, Reference number [redacted] she reviewed his claim and
found that there was no second billing to [redacted] from Butler Memorial Hospital. The reason for the
amount to increase was [redacted] did a review of his claim and found that he did not have Medicare

• Bill was sent to patient 11/1/13
• Mr. [redacted] called Butler Memorial Hospital on 12/2/13 we told patient to call [redacted] to
discuss why claim was denied.
• On 12/11/13 Mr. [redacted] contacted our office to ~et us know he received a letter from [redacted]
stating the claim was processed for payment.
• On 12/11/13 we did received payment from [redacted] with patient owing $640.29 coinsurance
• Mr. [redacted] began making monthly payments of $50 starting in January 2014 to Butler Memorial
Hospital. Total amount paid by Mr. [redacted] so far is $400
• On 7/16/14 [redacted] retracted their original payment and repaid Butler Memorial Hospital at a
lower rate stating now the patient owes $960.38 coinsurance.

My 3 year old nephew had to be taken there after he fell and cut his head open. We waited 90 mins to be seen after numerous people who came in after us and less severe were taken before us. When we asked the front desk told us we were next. Then 4 more people were sent back the nurse came over and told us we were not a priority. Yet blood was rushing from the wound on his head he was dizzy and his nose started to bleed. Finally we were taken back @ 10: 30 at night and the woman who took us back tried to explain again how him bleeding from his head and his other symptoms were not serious. After some choice words were exchanged between us and her she told us that we should probably find another hospital to go to next time we have an emergency as she used air quotes. No wonder this hospital is rated one of the worst ones and people said we were better off going to a chop shop instead of butler memorial hospital. Take my advice and drive the 40 mins to passavant. These people don't care and definitely don't make you feel like you are cared for. Also my nephew had to be rushed to another hospital a day later from passing out and ended up have a severe concussion. Guess if he was treated like a priority and they took the time to do tests they would have found it.

Review: I have been paying monthly on a Butler Hospital bill since July 25,2013 and didn't get a bill for August so I called Butler. I was informed by Butler that my bill was resubmitted after one year and my new balance had gone up by $320.09. My balance as of August was only suppose to be $240.29.Now the new balance was $560.30. How can a hospital bill go up over $300.00 after one year? I questioned Butler "WHY" was the bill resummitted and "WHY" did it increase by $320.09? I got no answer then they blamed it on [redacted]. I called [redacted] and they blamed it on Butler. I would like an answer to both questions. Please help!!! Desired Settlement: I would like the extra charges of $320.09 taken off of the total amount owed. So I would owe $240.29.
