To Whom It May Concern:May19, 2015This letter is in response to a recent correspondence from your offices regarding a patient's account.The patient had an estimate for services that were to be rendered for dental treatmentAs with each case, weexplain to our families that the
estimate is based on the insurance benefits we have been quoted by an insurancecompany representative, but that there is no guarantee how the insurance companies will pay until they haveactually received the claimWe were expecting full benefits to be available to pay for the dental services, but whenthe insurance company actually paid, it was less than anticipated because a portion of the available maximumbenefits had been used by the patient at another facility unbeknownst to us.There were at least three (3} billing statements mailed and at least two (2) phone calls documented from the lastfour (4) months, which shows that there was sufficient documentation sent to the residence regarding the accountbalanceNothing was returned to us, so we're assuming the address on file was correct.The patient's mom used her revolving balance Care Credit account for the treatment appointmentThe patient'smom brought up her Care Credit during the 05/07/phone callAs an option, I offered to transfer the balanceto her *** *** which would be an interest free option for the balance as we would have to charge her aninterest rate at our office if she did not pay off the balanceTherefore, we have not accessed her care creditaccount since the date of treatment on 1/10/The information we have is on file from that date, thus notviolating any HIPAA rights.The patient's mom wanted to pay $a month for the balance of $In certain instances, we understandpayment arrangements are necessary and we have no problem working with our patient's family as long asreasonable monthly payments are madeUnfortunately, $a month would take almost two years to pay t hisbalance in-full and we cannot accept such an arrangement at t his time.Also, it was the patient's mom who brought up collections, not usHer statement was that if $a month wasn'tgood enough, we could send her to collections.We would be happy to make a reasonable arrangement with our patient's familyWe just need our families towork towards a mutually acceptable arrangement.Sincerely,Butler Pediatric Dentistry*** ** *** ***, MSD ECHAPMAN AVE, #:226FULLERTON, CA 92831* ***

Review: My daughter was seen for a procedure in Dr.Butler's office back in Januray. I was treated nice and professional. They gave me a quote based on my daughters benefits. Which was checked at the time of service. And advised what they will get paid and what my portion would be. Three months later I receive a bill for almost five hundred dollars and was told I have been notified of this with several phone calls which I have not received. I let them know my current financial situation but her office staff was very rude and did not care. Told me I am responsible for the balance and I will go to collections. She also told me to charge on my carecredit which I NEVER asked her to check if I have credit. This account does not belong to their office and this is a violation of my privacy. They are only allowed to check when permission is granted. I will be looking into HIPPA violations for that. So they promise you the world but once they have you treat you like dirt. This should not be acceptable since I did not decline to pay even though they checked how much my insurance would pay back in January...Desired Settlement: I feel an apology is owed to me and they should re train their financial person to tret pt's with respect and dignity! If a pt calls upset you don't just turn them away TRY to find a solution to the problem...
