Ms. [redacted] as a patient in my practice, was referred by her PCP for Invisalign treatment in 2012. The treatment was successfully...

completed in June 2014. Ms. [redacted] received a set of retainers. To my knowledge and understanding she was very happy with the outcome of the treatment.
I did see her back in September 2014 regarding a concern with her lower left area. My diagnosis was; tooth #20 needs a re-treatment of previous root canal or extraction. Ms. [redacted] did not agree with my option of extraction, therefore I referred her to an Endo specialist for a second opinion. The specialist I referred her to diagnosed a root fracture. He also recommended extraction.
After both diagnosis Ms. [redacted] took the opinion of a friend and saw another general dentist who then referred her to a periodontist. She planned on seeing the periodontist every 3 months for cleanings. I had always suggested to Ms. [redacted] to have 3 months recall for cleanings but she refused to do so because her insurance only pays for every 6 months.
I have treated Ms. [redacted] for over 4 years. She has refused recommended treatment due to lack of Insurance coverage, but our dental care provided to Ms. [redacted] has always been quality dental care.
I have dealt with this same complaint thru Delta Dental and they had the opportunity to review the complaint and all the records of treatment. They have determined there is no refund for any portion of the monies paid for Invisalign treatment.
Ms. [redacted] filed an appeal with Delta Dental. They again had a chance to review the information and have decided that the earlier decision stands.
Thank you for your attention to this matter.
Sincerely,
Dr. [redacted], DDS
