received collections letters for their client [redacted] on accts [redacted] and [redacted] regarding [redacted]. The first bill is for me, however I paid this amount in full (original bill was $1500 and I made monthly payments). The last several payments were cashed and not applied to the correct accounts. I appealed with [redacted] repeatedly and they never responded but have now sent to a collection agency. The second account is for a minor child that I did not accompany to the ER, nor did I sign financial responsibility for. Also, their were unbundled ER charges (billing fraud) that I have reported to the Arizona Department of Insurance. I have NEVER re
