I have been a loyal donor for over 10 years may be close to twenty years and they took a phone call from possibly my ex-girlfriend or an affiliate saying my address changed and I have an ex parte restraining full order of protection against her and she isn't supposed to harass me in any way. I wasn't aware CSL was divulging that. I wasn't allowed to donate Monday 8-13-18 and they refused a piece of mail from July.
Attached are the HIPPA form and original complaint.

CSL Plasma followed all procedures in regard to privacy. The person that called the center simply stated that he wasn't at the address given to us, we never confirmed that Kevin was a donor. Following our SOP, we are required to ensure that donors have a valid current address before donating, which we did in this case. To verify that we asked Kevin to provide proof of residency which required something with a current date. Kevin was unable to provide proof of address that was current. Once proof of address has been provided with a current date, he can return to donating. This information was shared with him when he returned to the center.

I traveled 25 minutes to get to the Concord Plaza location only to be turned away because of my age. No where on their website is there any upper age restriction. I am 65.
