For the record - this notification is the FIRST one I have seen from the Revdex.com.The patient was seen as a new client on 11/20/14, given a thorough consultation which included risk, benefit, dosing and typical duration of result for Botox cosmetic. Her appointment was 15 minutes long, yet was given over...

30 minutes answering questions and discussing treatment options. She verbalized understanding, signed the treatment consent form and requested treatment.The patient wanted to spend $50 for her treatment and was advised how many units that would purchase and that it was NOT typical or customary to use only 5 units in the area requesting to be treated. She was advised that first time Botox users may have a result lasting 3-6 months (with RECOMMENDED dosage of 20-45 units/$250-450). She VERBALIZED UNDERSTANDING AND AGREED to allow 10 units/$100 of Botox to be used (still NOT the recommended minimum of 20).She was given our customary 2 week follow-up appointment, free of charge, where she verbalized complete satisfaction with result and requested an additional area be treated! It was noted that the initial treatment area was sufficiently treated and the asymmetry she was pointing out was due to shape of eyebrow and not Botox result. She agreed and had no further complaint about the initial treatment area and received treatment in an additional area.She was again advised that because of the low dose requested for both areas, she may have a good result at onset but that it may not last. She VERBALIZED UNDERSTANDING.The patient called the office on or about 12/19/14 (4 weeks post initial treatment), requesting an appointment to have a 3rd area done! At that time, she made no mention of being dissatisfied with previous 2 areas treated. However ultimately she did not make an appointment claiming her schedule did not allow it.On 1/6/15 (7 weeks post initial treatment), the patient called and stated she was not satisfied with her treatment. She was offered a follow-up appointment and advised of the appointment charge - "a consultation fee of $25 plus any additional Botox cost, as indicated." I was present during the phone conversation.The patient verbalized she did not want to pay for either the office visit or any further Botox treatment. The patient's interpretation of how the conversation went is entirely hers, however she was offered a follow-up appointment and an opportunity to receive more treatment but refused.In my professional opinion and with my patient notes of her visits, she was given more than adequate consultation time, given treatment at her request with substandard dosing while being informed it may not last, an opportunity to have her challenges discussed with me, and the option of being treated with more Botox. She refused. It is not customary to see a medical provider for any reason without at least a "co-pay" and her treatment results were exactly as she was informed they would be.Therefore, her desired settlement of a full-refund or re-treatment at no cost will not be considered.
